Before Pendragon Forms:

We are a metabolic disease service at Mount Sinai hospital with around 400 patients in both outpatient clinics and on inpatient floors.  We see patients regularly and need to manage information on medical conditions, medications, special nutritional formulas, demographics, insurance, and lab results. 

 

Before using Pendragon Forms, all of this information was scattered in various computer and paper systems.  There was also no good way for physicians who were paged in the middle of the night to access data.

Installation:

The major form has entry demographics and a diagnosis page for each patient.  Using subforms and lookups, a new record is created under the main page for each visit to the clinic or for each inpatient day.  A medical note is facilitated using multiple popup lists.  Varying formats of the note exist for different types of visits.  The notes can  be printed out for the charts with an IR printer. 

 

We also have separate forms for all the specialized medications we use and for the specialized formulas.  This form can calculate the exact amount of fat, protein and carbohydrates and many other nutrients that a patient is getting based on the amount of formula that is taken.  This form can actually be accessed from the main visit form and the information prints out with the note.

  • Lots of information is now available instantly that was difficult to get before.  
  • Missing charts are no longer a problem. 
  • Complicated calculations of nutrients are done automatically. 
  • Notes are easier and partially automated. 
  • We have information available when at home on call.
Estimated ROI: Although it is impossible to estimate the financial benefit, we all feel that it has helped us and our patients.
