Pendragon Forms is used by students to collect information on surgical and clinical procedures in Surgical Physician Assistant Program required to maintain licensure and ensure completion of program requirements.

Forms Development Time:

Developing individual forms was very quick even for a person with “very little computer savvy”.  After faculty tested forms for one month, the complete program comprised of four forms was in the hands of students. Over a twelve month time period, four different versions of forms were deployed.

 

Installation:

Twenty five students in the Surgical Physician Assistant Program use four different forms to track their activities on handheld devices as they see up to fifty patients each day.  Each form ranges from four to twenty one pages and is used to collect data on surgical operations, procedures outside the operating room, patient assessments and conference attendance.

 

Students synchronize their handhelds in a nearby computer lab where four workstations have been equipped with sync cradles.   Data from Pendragon Forms is sent via intranet to a database that can be accessed from Kloda’s PC.  Built-in security features have been selected so that students cannot change or review their procedure data.  After synchronizing, all information is erased from the handheld. 

 

Before Pendragon Forms:

Students completed a monthly log of procedures by hand.  There were many omissions and incomplete data due to reliance on memory.

Benefits:

It is very easy for students to gather complete and accurate information “on the run” in both clinical and surgical settings.  Capturing data using Pendragon Forms has simplified the record keeping requirements for licensure and reduced the amount of time spend each month collecting, collating and filing paper records. The database has enabled analysis of individual, service and program information.
